About the Role

As a Qualified Mental Health Professional (QMHP-CS) at Pacesetters Home Health, you will work directly with clients to promote emotional wellness, independence, and daily living success. You will:

  • Develop individualized, person-centered treatment goals and coping strategies.
  • Deliver skill-building interventions, psychosocial education, and parent support.
  • Provide Targeted Case Management (TCM) services by connecting clients and families to community resources and coordinating care among providers.
  • Maintain accurate, timely documentation in compliance with Texas Administrative Code, TMHP, and managed care organization (MCO) standards.

Qualifications

  • ILINGUAL (SPANISH) AND English-speaking Preferred
  • QMHP-CS A staff member who is credentialed as a QMHP-CS who has demonstrated and documented competency in the work to be performed and has:

A minimum of a bachelor’s degree from an accredited college or university with a minimum number of hours that is equivalent to a major (as determined by the LMHA or MCO in accordance with §412.316(d) of this title [relating to Competency and Credentialing]) in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention;

Responsibilities

  • Provide comprehensive behavioral health and case management services to the client in accordance with the treatment plan that is designed to meet the mental health, educational, vocational, residential, financial, social and other needs of the client and their family.
  • Provide Skills Training and Development based on individualized recovery plans in collaboration with clients and legal guardians.
  • Collaborate with clients, legal guardians, and collective staff to update recovery plans regularly and when needed.
  • Provide crisis intervention support when needed.
  • Maintain accurate and up-to-date documentation of all services provided through progress notes and contact logs within 48 hours of the date of service for review.
  • Follow Texas Department of State Health Services, Resilience and Recovery Guidelines and ensure that treatment plan authorizations and reauthorizations, CANS/ANSA and updated assessments DO NOT expire. Must be willing to become CANS/ANSA certified.
  • Serve as an intricate member of the Pacesetters Home Health team to ensure continuity of care and support the services 24 hours a day, seven days per week as necessary
  • Provides structured face-to-face scheduled sessions to clients based on the treatment plans.
  • Provide services in the home, school, office and community setting
  • Ensure that the majority of mental health services are primarily rendered in the community and ensure that limited services remain clinic-based
  • Documentation completed within 24 hours of service or phone call
  • Report to supervision and in-house training as scheduled
  • Work collaboratively with and provide ongoing updates to the clinical team including the psychiatrist as needed
  • Identify, establish, and continually collaborate with community agencies, providers, and natural supports and advise team to make referrals as needed
  • Listen/respond to; interact with clients, co-workers, management, and other professionals in a non-judgmental manner with a team-based approach
  • Communicate objectively, thoroughly, accurately, and concisely in a manner consistent with the cognitive level of the persons involved
  • Demonstrate an understanding and acceptance of the organizational mission and shared purpose to assist individuals by providing quality services
  • Participate in efforts to enhance service provision to consumers
  • Act and interact to build cohesiveness
  • Assist with building a positive image for the agency by demonstrating support and cooperation with other units and community agencies
  • Demonstrate professional work ethics as evidenced by overall dependability, conscientiousness, and timeliness

Benefits

Hourly Rate Recommendation: $24 – $28 PER HOUR
FLEXIBLE SCHEDULE

Requirements:

Must become QMHP-CS CERTIFIED in accordance with state and federal requirements and complete all required training.

A staff member must meet at least one of three minimum requirements to be credentialed as a QMHP-CS:

(1) The staff member has at least a bachelor’s degree from an accredited college or university and a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention.” Legal Information Institute

(2) The staff member is a registered nurse. Legal Information Institute

(3) The staff member is a Licensed Practitioner of the Healing Arts (LPHA).

Proof of valid Texas Driver’s License (Type C), Current auto insurance, access to reliable transportation, and good driving record

  • Bachelor’s Degree in Human Services or related field (required)
  • Minimum two (2) years behavioral health experience (preferred)
  • Must meet HHSC QMHP requirements
  • Valid Texas Driver’s License (Type C)
  • Reliable transportation and proof of current auto liability insurance
  • Consent for criminal background check
  • Cleared TB test (within 12 months)
  • Consent for Pre-Employment Drug Screening
  • Working cellular telephone at all times for on-call and after-hours availability

Hourly Rate Recommendation $26.00 – $28.00 per hour

Qualified Mental Health Professional - Community Services (QMHP-CS)

QMHP-CS providers are eligible to deliver MHTCM and MH rehabilitative services and must meet the following minimum credentialing requirements:

  • Completed a standardized training curriculum
  • Demonstrated competency in the work to be performed
  • Obtained one of the following:
    • A bachelor’s degree from an accredited college or university with a minimum number of hours that are equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention
    • A license as a registered nurse (RN)

Staff administering the functional assessment instruments must have documentation of current certification in either the Child and Adolescent Needs and Strengths Assessment (CANS) or the Adult Needs and Strengths Assessment (ANSA). Certification in either the CANS or ANSA must be updated annually through an approved entity.

An individual who possesses any of the following licenses is considered a Licensed Practitioner of the Healing Arts (LPHA) and is automatically certified as a QMHP-CS:

  • Physician
  • Physician Assistant
  • Advanced Practice Registered Nurse
  • Psychologist
  • Licensed Clinical Social Worker (LCSW)
  • Licensed Marriage and Family Therapist (LMFT)
  • Licensed Professional Counselor (LPC)

A QMHP-CS must be clinically supervised by another QMHP-CS. If a QMHP-CS is clinically supervised by another QMHP-CS, the supervising QMHP-CS must be clinically supervised by an LPHA.
